Nicholas Marriott 5f6a351df7 Start the first client with a special socketpair so it is already known to the 
server rather than playing silly games to get them synchronised before doing
anything.

Change attach-session to start the server.

Nicholas Marriott 97fef895f2 Rework the prefix-time stuff. The option is now call repeat-time and defaults to 500 ms. However, it only applies to a small subset of commands, currently: up-pane, down-pane, next-window, previous-window, resize-pane-up, resize-pane-down.
